a further improvement may be obtained in survival rate. The disease, starting in the cervical canal or on a lip of the cervix, spreads by direct extension into the paracervical tissues and by lymphatic drainage into the broad ligaments and regional lymphatic glands. This spread of the disease may be either generalised or confined to one or other fornix with or without spread on to the vaginal wall. If necrosis and sloughing take place then a large excavating ulcer develops with seminecrotic tissue forming its base. If, on the other hand, infiltration is more rapid than ulceration then a marked stenosis of the upper part of the vagina results from the surrounding growth of neoplastic tissue. Frequently, however, the disease takes on the form of a large hypertrophic mass growing from one or other lip of the cervix and filling up the vault of the vagina, exhibiting in addition the other manifestations of intrapelvic extension.
